CFN 2018 National Conference -- Improving Frailty Care Benefits Everyone

Frailty matters. 1.2 million Canadians 65-plus live with frailty as do 2.5 million Canadians who are their caregivers and hundreds of thousands more in our healthcare workforce.

CFN's 2018 National Conference offered attendees opportunities to learn, network, brainstorm new ideas, address key questions and challenges at Canada’s only conference on the complex and critical issue of frailty.

This conference is for older Canadians living with frailty and their family/friend caregivers, clinicians, researchers, students and trainees, health and social care providers, policy decision makers and experts in the field of frailty.  Together we will discussed the latest knowledge, evidence, approaches and policies that are transforming care and seeding the next generation of breakthrough innovations.

CFN is Canada’s only national network dedicated to older Canadians living with frailty and all responsible for their well-being. The Network has funded and catalyzed new research, knowledge, practice, education and training across the full continuum of care. This conference is a unique opportunity to examine frailty from many aspects/perspectives at a single event.

A centrepiece of the 2018 CFN National Conference, the FRAILTY MATTERS Innovation Showcase put the spotlight squarely on the best emerging or existing innovative approaches to improving the lives of older Canadians living with frailty and all responsible for their care. The CFN FRAILTY MATTERS Innovation Showcase was presented in partnership with the Canadian Foundation for Healthcare Improvement (CFHI) and the Canadian Institutes of Health Research -- Institute for Aging (CIHR-IA).

And innovation is alive and well in Canada, as witnessed by the 80 submissions we received!  The top 30 innovations exhibited storyboards at the 2018 CFN National Conference, and the top five presented their innovations  in a plenary forum, with the Top Canadian Frailty Innovation announced from stage.
