2019 Knowledge Translation Grant — An innovative decision tool to optimize hospital discharge in frail older patients living at risk (KT2019-10)

Dr. Véronique Provencher is a Professor at the School of Rehabilitation (Occupational Therapy), Faculty of Medicine and Health Sciences, Université de Sherbrooke. She is a Researcher at the Research Center on Aging and holds a provincial (Junior 1 FRQS) Research Fellowship. She is also Associate Editor for the world’s highest-ranking journal in her discipline (American Journal of Occupational Therapy). Dr. Provencher’s research is focused on risk assessment in older adults living with frailty before and after hospital discharge.
