Taking the pulse of the nation: CFN cross-country collaboration tour

CFN is re-engaging with citizens, researchers, partners and policy makers to refine our understanding of their needs, to incorporate in our strategic planning. CFN has finalized an ambitious plan and agenda for 2019 with committed partners to visit cities across Canada such as: Yellowknife, Prince George, Vancouver, Calgary, Toronto, Ottawa, Montreal and Halifax. The plan is to canvas these individuals and organizations so that we better understand how frailty impacts their communities.

Goal: CFN aims to improve care for older adults living with frailty by increasing the recognition of frailty in all settings of care, implementing evidence- based care practices and improving system integration.

Follow CFN as we engage with Canadians across the country! Please join us if we are in your area.
