Call for proposals open. CFN National Conference on Frailty 2019: Innovation and Collaboration for Impact
Frailty matters. 1.2 million Canadians 65-plus live with frailty, as do 2.5 million Canadians who are their caregivers and hundreds of thousands more in our healthcare workforce. CFN is Canada’s only national network dedicated to older Canadians living with frailty and all responsible for their well-being.
The Canadian Frailty Network (CFN) is inviting innovators to submit their innovation for an opportunity to be highlighted at the National Conference on Frailty on September 26-27 at the Delta Marriott Toronto Airport Hotel. Submissions should describe demonstrated or emerging impact on health or social programs, policies or initiatives focused on improving the lives of older Canadians living with frailty and their friend/family caregivers. From all submissions, CFN will be inviting a select number of innovators to present their projects in either oral (podium) or storyboard form.
The Canadian Frailty Network and the Canadian Foundation for Healthcare Improvement are working together with the Top 5 innovators from the 2018 CFN FRAILTY MATTERS Innovation Showcase to explore how their innovations can be spread.
