CFN 2018 Frailty Innovation of the Year Announced

CFN created the FRAILTY MATTERS Innovation Showcase to put the spotlight on Canada’s best innovations to improve care for older Canadians living with frailty. A new idea taken back to your organization, or passed on to a care provider, policy-maker, citizen, advocate, caregiver or patient could be the seed to fuel improved care in your area.

The Showcase competition drew 80 submissions from every region of the country, representing nearly every conceivable facet of care for older adults living with frailty.

An Edmonton-based community program designed to transform primary care into a central hub has been selected as the best frailty innovation of 2018.

The Seniors’ Community Hub (SCH) was co-created by Dr. Marjan Abbasi (project lead) and Dr. Sheny Khera to better meet the dynamic health and social needs of older adults with frailty and their family/friend caregivers.
