2018 Catalyst Grant Program — “C5-75”: A primary care program to identify and support older adults living with frailty (CAT 2018-19)
Dr. Linda Lee is a Care of the Elderly Family Physician, Schlegel Research Chair in Primary Care for Elders at the Schlegel-UW Research Institute for Aging, and Associate Clinical Professor, Department of Family Medicine, McMaster University. In 2012, she led the development of the C5-75 model of care at the Center for Family Medicine Family Health Team in Kitchener, Ontario. Dr. Lee has also been recognized for developing the innovative Primary Care Collaborative Memory Clinic model of care and its interprofessional training program, helping to establish Memory Clinics in 107 primary care locations across Ontario. For this work, she was awarded the 2014 Ontario Minister’s Medal Honouring Excellence in Health Quality and Safety and the 2015 Canadian Institutes of Health Research (CIHR) Institute of Aging Betty Havens Award for Knowledge Translation in Aging.
